The following issues have to be resolved ASAP: 1) The account is in the name of Financial Trust Company (not Jeffrey). The Tax Id # ending in-s that of Jeffreys not of Financial Trust Company. Also. Jeffreys name is on the 1099. 2) a corporation is exempt from Forms 1099 reporting requirements. Therefore this erroneous 1099 should be voided (not corrected). Under separate email I will forward to you the corrected statement that I received. (use the following password to open Ma. it doesn't appear to be a 1099, and the correction shows income items in this account). Again- the erroneous 1099 sent with Jeffrey's name and tax INS should be voided and NO 1099 should be issued for Financial Trust Company. 3y attn Paula Sherman" that appears on the account (both on monthly statements as well as on the erroneous 1099 and the 2010Tax reporting Statements ) must be removed.
