On this web site is a database of all entities who have registered for the "CICI identifier". The database shows the name, address, entity form and CICI identifier for each registered entity. JP Morgan is requesting that we provide a CICI identifier for FTC by December 1,2012. The registration fee is $200 per entity. A $100 yearly maintenance fee will also be charged.
