They will prorate=the premium so they need a check for $1,149.50 for your insurance which wi=l run from November 1, 2015 to September 30, 2016. She was supposed =o email me the forms for you to fill out but has not done so as yet and l'=e called her back several times to remind her. The cap on the insura=ce is $250,000, which is what is required for the VI.
