I will need the following in order to purchase the insurance and finalize the arrangements: • Personal Information Form (one each from Mattie & Miasha) • Assumption of Risk (one each from Mattie & Miasha) • Trip Waiver (one each from Mattie & Miasha) • Credit card authorization form for payment of insurance I also need to know if you would like for me to process your Kenya visas.
