From: To: Subject APPIII ottabk p e n mduttkn) Date: Attacbsnentt Friday, August 9, 2019 3:52:15 PM Wahin On August 9,2019, at approximately 1:37 pm., while conducting routine duties, the 9 South Visiting Officer observed a female visitor produce an unknown object from her waistband and hand it to *nmate Fernandez Leonardo Reg. No. 86824.054. Staff immediately entered the visiting room and instructed inmate Fernandez to stand, and he was escorted from the scene to search area without further incident. Inmate Fernandez was instructed to relinquish the item, however inmate Fernandez continued to deny he was in possession of any contraband. Inmate Fernandez was photographed and medically examined and with no apparent injuries identified. Inmate Fernandez was visually searched with negative results and provided a urinalysis test. During the test inmate Fernandez poured water into the testing cup in attempt to not provde a urine sample. Inmate Fernandez was placed on dry cell status. The visitor was escorted to the lobby where she was detained and questioned and later released. There were no staff injuries reported.
