FD-302 (Rev. 5-8-10) -1 of 2- FEDERAL BUREAU OF INVESTIGATION OFFICIAL RECORD 1 DateofetUry 12/19/2019 On August 21, 2019 at approximately 0845 hours the undersigned, along with Special Agent David Carpenter of the Office of the Inspector General, Special Agent Amanda Moore of the Federal Bureau of Investigation and Assistant United States Attorneys for the Southern District of New York Rebekah Donaleski and Nicholas Roos did interview Federal Bureau of Prisons Psychiatrist District of NY. EMPLOYEY , at the Southern f informed that she has been with the BOP since 07/17/2000 and that sne has been the staff psychiatrist since 2002 advised as to the procedures and policies in place at the MCC for her role and how inmates meet with the psychiatrists. 07/23/2019 and conducted a suicide risk assessment. According to EPSTEIN had no clear recollection of the incident. EPSTEIN went on to even tell that he is "too much of a coward" to commit suicide, that he "doesn't like pain" and that suicide goes against his Jewish religion. Also according to EPSTEIN had many normal interactions with those in the psych unit and even those guards that were assigned to observe him. did advise the staff that EPSTEIN would need a cellmate. When the psych staff reviewed EPSTEIN's file in their routine meetings EPSTEIN yielded no factors that would have placed him back in suicide watch. stated that had she or her department been notified that EPSTEIN changed his will that that would have been a factor that would have warranted a review of his mental state and would have probably been placed back in suicide watch.
