Dear Quest Diagnostics Patient,
Quest Diagnostics recently submitted a claim to UnitedHealthcare for laboratory testing performed on your behalf.
As a result of the claim, you may have received an Explanation of Benefits (EOB) from UnitedHealthcare indicating
that the testing is an Out-of-Network benefit. The EOB may also have included a patient responsibility amount due to
Quest Diagnostics.
The Out-of-Network benefit detennination by UnitedHealthcare may differ from your prior claim experience when
receiving testing by Quest Diagnostics. As such, Quest Diagnostics appealed the claim(s) for your testing with
UnitedHealthcare. They upheld their decision, we believe, because of a recent reimbursement change implemented
solely for Fully Insured members in select UnitedHealthcare markets.
In light of the uncertainty with your benefit coverage at the time the testing was ordered, Quest Diagnostics will
extend to you a one-time exception to its general billing practices in Florida and forgive the patient responsibility
amounts listed above. As a result of this action, the balance for the date of service(s) indicated above reflects a zero
balance.
