I'm not sure how anyone could question the name and address of the business after seeing all of these items. It is my Certificate of Authority, 2 letters from the IRS stating the company name, address, EIN number, my Business Certificate and several invoices from vendors which are to the trade only which I have purchased fabrics, furniture, etc from for my projects.
