On Tax form SSN and EIN numbers are given, however individual is filing tax for entity so it should be SSN number given on tax form. 2 On tax form , Darren Indyke address is given as 2 Kean Court Livingston NJ 07039 USA, however on tax form address is given as 575 Lexington Avenue, e fl, New York, NY 10022, both are different. 3 On Signature card, Entity type is not selected.
