PRIMECARE The Choice for Quality Correctional Healthcare January 16, 2014 Warden Richard Smith Centre County Prison 700 Rishel Hill Road Bellefonte, PA 16823 RE: Annual Cost-of-Living Increase Year #5 Health Services Agreement Dear Warden Smith: As outlined on Page 3, Item 5 of the Comprehensive Health Services Agreement between Center County and PrimeCare Medical, Inc., the annual compensation for services increase is based upon the September Cost-of-Living Index for the US. City Average of Medical Care Services as published by the United States Department of Labor. The previous twelve months Cost-of-Living Index ending in September 2013 was 3.1% (attached), therefore the increase for the contract year January 1, 2014 through December 31, 2014 will be The 3.1% increase contractually translates as follows: $697,275.36 (Year #4 Annual Cost) 3.1% $718,890.90 (Year #5 Annual Cost) 58,106.28 (Year #4 Cost) 3.1% $59,907.58 (Year #5 Cost) The above increases are effective January 1, 2014 and will be re?ected on your January invoice.
